Rice, boiled. Noun first, so ต้ม is working as an adjective — rice-that-has-been-boiled — and the grains stay whole, which is what separates it from โจ๊ก.
Word order is doing real work here. As the first word of a name ต้ม means a soup and brings its liquid with it (ต้มยำกุ้ง, ต้มข่าไก่); after a noun it only says the thing was boiled, which is why ไข่ต้ม is a boiled egg and ข้าวต้ม is boiled rice. The name does not distinguish the one-bowl version from ข้าวต้มกับ, plain rice soup surrounded by small shared dishes.
